Welcome Packet, Policies & Forms
All owners of pets participating in Daycare or Boarding must fill out a Welcome Packet prior to boarding or daycare stay. You may download, fill out, and email a .pdf copy to Happypets@dogtownkittycity.com or print and bring a copy into our facility. We do have printed copies available for pick up as well.
All pets must be vaccinated according to our vaccination policy in order to participate in any services we provide.
